Factors Influencing on Length of Stay in Closed Wards of a Municipal Mental Hospital

Abstract
Objectives:Chronic hospitalization is still prevalent in Korea against the recommendation to reduce the number of mental hospitals and activate community mental health system by World Health Organization. Length of inpatient stay(LOS) is an important factor to determine which is cost-effective system to reflect the appropriateness of admission and to raise the quality of life in psychiatric patients.

Methods: Data analysis has been done on 83 inpatients who consecutively admitted to psychiatric closed wards in a municipal hospital to assess the variables which would most likely predict LOS. The variables included sociodemographic characteristics, psychiatric diagnosis, duration of illness, PANSS, CGI, family and friend APGAR and life event questionnare.

Results: The mean LOS for the 83 patients was 65.9 days(S.D 54.2). Patients with diagnoses of psychotic disorders and organic disorders were hospitalized for longer periods than were patients with mood disorders and alcohol use disorders(p<0.01). LOS has been determined significantly by the type of reimbursement(p<0.05). In multiple regression analysis, the best model including diagnosis, type of reimbursement, duration of illness, family APGAR accounted for 33.1% of the variation in LOS, only significant in diagnosis(F=4.56, p<0.01) and type of reimbursement(F=5.06, p<0.01).

Conclusion: Diagnosis and type of reimbursement should be considered to be important variables in predicting LOS. The following study would be warranted to replicate this result and find out other factors to influence the LOS.
